Sioux Center Health is one of 12 Iowa hospitals included in Forbes’ inaugural 2026 Top Hospitals by State list.
The other Northwest Iowa hospital on the list is Floyd Valley Healthcare in Le Mars.
The state list is a subset of Forbes’ national Top Hospitals evaluation. Hospitals had to meet the national evaluation criteria and receive an overall star rating to be eligible.
Forbes then used star-rating percentiles to select approximately the top 30% of eligible hospitals in each state. Hospitals receiving one- or two-star overall ratings were excluded, while all five-star hospitals were included. Ties at the cutoff were also included.
The final state list recognizes 781 hospitals nationwide, averaging 15 hospitals per state.
“Truly great healthcare begins with great people. This recognition is a significant honor because it is a reflection of our extraordinary team,” Sioux Center Health CEO Cory Nelson said.
Nelson also credited the hospital’s technology, staff expertise and relationships with the communities it serves.
How Sioux Center Health Scored
Forbes considered as many as 56 healthcare-quality measures, although not every measure was available or applicable to every hospital. The Sioux Center Health profile displays 23 individual measures.
The hospital received three stars for outcomes and four stars for best practices and patient experience. Its available value measure, based on the amount Medicare paid for pneumonia cases, received four stars.
Six individual measures received five stars. Those included influenza vaccination among healthcare personnel, nurse communication, patients receiving prompt help, and inpatients rating the hospital nine or 10 on a 10-point scale.
The measures covering room quietness at night and patients who left the emergency department before being seen also received five stars.
Four-star measures included hospital-wide readmissions, cesarean births, severe obstetric complications and the time patients spent in the emergency department.
Sioux Center Health received one star on two patient-experience measures: whether staff always explained medications before dispensing them and whether patients reported their rooms and bathrooms were always clean.
Several outcome measures received two stars, including the 30-day death rate among pneumonia patients and unplanned hospital days after treatment for heart failure or pneumonia.
How Forbes Calculated the Ratings
The ratings are based on healthcare quality measures from the Centers for Medicare & Medicaid Services Provider Data Catalog.
Patient outcomes account for 55% of a hospital’s overall rating. Best practices make up 20%, value accounts for 15%, and patient experience represents the remaining 10%.
Forbes says the outcome measures were statistically adjusted for clinical health risks and social drivers of health. The adjustments were intended to provide a more even comparison among hospitals serving different populations and geographic areas.
The Sioux Center Health profile cites CMS quality-measure results published in April 2025, using information including Medicare claims, chart-abstracted data and patient surveys.
